Shows date of death; date of last admission; last serial status number in civil register (reference number as a voluntary, temporary or certified patient at Runwell Hospital); general reference number (as a patient at Runwell Hospital); name of patient; sex; age at death and at onset of last 'attack' of mental illness; classification (i.e. whether private, service, criminal or rate-aided); whether a voluntary, temporary or certified patient on admission and at death; whether 'attack' was congenital, first attack, not first attack or unknown; duration of last 'attack of mental illness' before admission, after admission and in total; causes of death; whether post mortem held; columns for aetiological factors and form of mental illness on admission, completed using alphanumeric symbols as scheduled by Board of Control; observations.

Signed annually until 1948 by Commissioners of the Board of Control and at intervals by others, latterly members of the Hospital Management Committee.

Admin History:
The patients whose deaths were recorded in this volume had been admitted between 1936 and 1953. However, only 6 of them (3 of whom died in November 1950, and one each in January 1952, June 1954 and May 1955) had been admitted after the inauguration of the National Health Service in July 1948. The first 3 such entries are annotated as having been made in error, and one other has been struck through. Probably the register was maintained after 1948 for pre-NHS patients only.
